WASHINGTON — October 5, 2026: President Donald Trump has announced a new one-time $90 Medicare payment for more than 20 million eligible Americans, with the administration saying the money is intended to help offset the cost of Medicare Part B premiums.
The payment is being described by the White House as a Medicare Improvement Fund Premium Rebate. According to the Centers for Medicare & Medicaid Services (CMS), eligible beneficiaries will receive a one-time $90 rebate in October 2026.
Who is eligible?
The payment is aimed primarily at people enrolled in traditional Medicare Part B. However, it is important to note that not every Medicare beneficiary qualifies.
According to the White House and CMS, people whose Medicare premiums are already being paid through Medicaid, as well as beneficiaries who pay an income-related monthly adjustment amount, are excluded from the rebate. Medicare Advantage enrollees are also generally not included in the payment.
When will the $90 arrive?
CMS says most eligible beneficiaries are expected to receive their $90 payment by direct deposit around October 8, while some beneficiaries will receive a paper check later in October.
People receiving paper checks should make sure the mailing address associated with their Medicare records is current.
The payment is intended to help offset Medicare Part B costs. The standard Part B premium for 2026 is $202.90 per month, meaning the $90 rebate represents a significant portion of one month’s standard premium for an eligible beneficiary.
Where is the money coming from?
The White House says the payments will come from the Medicare Improvement Fund, a fund created by Congress in 2008 to support improvements to Medicare’s traditional fee-for-service program.
The administration says the fund contains about $2 billion and is now being used to provide direct financial relief to eligible Medicare beneficiaries.
The move has attracted political attention because it comes only weeks before the November 3 midterm elections. Trump has also promoted other proposed or ongoing payments to Americans, making the Medicare announcement part of a broader debate over the administration’s approach to providing financial relief.
Democrats and some policy analysts have questioned the timing and use of the Medicare fund, while the Trump administration argues that the money should be used to directly benefit Medicare beneficiaries.
What seniors should know
The $90 payment is a one-time rebate, not a permanent increase in Social Security or Medicare benefits. It also does not permanently reduce the Medicare Part B premium.
Beneficiaries who want to check whether they qualify can contact 1-800-MEDICARE (1-800-633-4227). CMS says people checking the status of a payment should contact the Social Security Administration at 1-800-772-1213 beginning October 15, 2026.
For millions of older Americans facing rising healthcare and living costs, even a one-time $90 payment could provide some short-term relief.
But with the midterm elections approaching, the announcement is also becoming a major political talking point — putting the spotlight on both the financial needs of America’s seniors and the political battle over healthcare costs.
